If you’ve been off sick for more than 7 days
If you have been off sick for more that 7 days please fill out a sick note request form by using the link below. A sick note will normally only be issued, if you have been seen by a GP or other medical professional whilst you have been off sick.

Requesting an additional sick/fit note
If you have already had a sick/fit note for this illness your doctor may not need to see you to issue an additional sick/fit note. Please complete the sick note request form for any additional sick/fit note. We will let you know when you can collect your sick/fit note or we may need you to arrange an appointment.
If you’ve been off sick for 7 days or less
If you’re off work sick for 7 days or less, your employer should not ask for medical evidence that you’ve been ill. Instead, they can ask you to confirm that you’ve been ill. You can do this by filling in a form yourself when you return to work. We call this self-certification.
